My husband asked me recently, "What are your goals for 2022?" I didn't even know where to begin! So I created the form attached for myself and thought it may be useful for you too! I added check boxes for quarterly check-ins because if you're anything like me, you will do AMAZING the first week or two then totally forget about your goals.
